New Patients

Please plan to spend approximately 1 to 2.5 hours at the office for your first appointment. We will review your medical history and check your glasses prescription.  All new patients will receive dilating drops in order to perform a complete eye examination.

 

You will need to bring:

  1. Current insurance cards and photo I.D.
  2. List of all current medications
  3. Current glasses
  4. Name of primary care provider (PCP)
  5. Records from previous eye doctors
